You'll need to submit your claims form to your AXA insurance agent or directly to AXA via mail. Click on "Personal" to view the entire list of claims that you can file, and you will be able to view the step-by-step procedure, and download the necessary claim forms. Do so ASAP after your diagnosis or your family member's death to avoid missing out on the claims window – you have to complete the claims usually within 30 days of the incident.

Submit To AXA
